Gentiva Health Services has been providing home health services nationwide for 40 years. Our comprehensive approach combines advanced clinical training with the latest educational tools including the Gentiva Health Education Center, our valuable online resource for residents, family members and clinicians. Gentiva works with the staff at Vista Oaks, the physicians, the residents and their families to develop a plan of care that best serves each resident’s needs. We utilize an interdisciplinary approach to help residents achieve the best possible outcomes and maintain the highest possible level of independence. Our team of nurses and therapists is dedicated to helping the residents continue to "age in place." Our objectives are to reduce hospitalizations, reduce emergency room visits, promote resident independence and improve the resident’s quality of life. Home health may be recommended if any of the following apply: The resident has a new diagnosis. The resident has new or changed medications. There has been a change in condition. The resident has difficulty performing activities of daily living. The resident has multiple risk factors that have the potential to impact their health. The resident has exercise intolerance or poor endurance. If any of these apply the resident’s physician may order home health.

Homecare is the foundation of what we do. Gentiva RN case managers coordinate all aspects of resident care from specialized procedures (such as wound care) to disease management. The case managers also assist in coordinating therapy (physical, occupational or speech) as appropriate. Our team at Vista Oaks is lead by Julia Deisler RN. Julia has over 20 years of home health experience and is committed to helping each resident receiving Gentiva’s services achieve the best possible outcomes. We are also pleased to have Brad Logan PT working with us. Brad is well known to the residents and families of Vista Oaks and is also dedicated to helping the residents maintain the highest possible level of independence.
